SunsetWell quality scores for every Medicare-certified nursing home in the United States. One row per facility with its CMS Certification Number (CCN), location, certified bed count, SunsetWell score (0-100, higher is better), within-state percentile rank, and the scoring model version. Scores are produced by the SunsetWell v2.3 predictive harm model, trained on CMS inspection, staffing, and clinical outcome data.

Federal enforcement summary for every Medicare-certified nursing home, covering the trailing three years of CMS inspection data. One row per facility with health inspection citation counts, whether the facility received an immediate jeopardy citation (the most severe finding, meaning residents were at risk of serious harm or death), substantiated complaint counts, and total federal fines in dollars.

Enforcement figures (citations, immediate jeopardy, complaints, fines) cover the trailing three years of inspection data, matching the window CMS uses on Care Compare. The median fine column in the state summary is computed among facilities that received at least one fine, since most facilities have none. State aggregates are derived directly from the per-facility files, so every summary number can be reproduced from the downloads on this page.
